NDIS invoice template

Free in Word, Excel or PDF, laid out for the five details the NDIA says a claim needs. Whether the invoice shows GST is a separate question with four conditions — and the instrument behind one of them has an end date.

NDIS invoice

Participant, NDIS number, date of support and support item number on every line. The Excel version adds up the lines.

Rather fill it in on screen? Fill it in below, and save the invoice as a PDF.

Set up for an NDIS claim: the participant and their NDIS number, then a date of support and a support item number on every line, with a check of the five details the claim needs. Save it as a PDF or print it. Nothing is uploaded or kept.

Your details

Logo (optional)

PNG, JPEG, WebP or SVG. Your browser reads it here; it is never uploaded.

The participant

A plan-managed participant’s invoice goes to the plan manager; a self-managed one’s goes to the participant. NDIA-managed supports are claimed in the provider portal instead.

The invoice
How you get paid (optional)

Printed under the totals as “How to pay”, with the invoice number as the reference. Terms set the due date from the issue date. None of this is required on a tax invoice, and none of it leaves your browser.

The supports

Each line takes one date and one support item number, so two supports, or one support on two dates, go on two lines. The number comes from the NDIS support catalogue, and the price is the one you agreed, which may be below the catalogue’s limit.

What the claim needs

The five details the NDIA lists for a payment claim. The plan manager or participant claims from this invoice, so it should carry all five.

Invoice checklist

This checks what is on the document. It cannot check that the details you typed are correct, and no invoice can be “ATO-approved” — the Act asks for enough information to be clearly ascertained, not a prescribed form.

What has to be on it

A plan manager or a self-managed participant claims the money back from the NDIS using your invoice, so it has to carry what their claim needs. The NDIA lists five things:

  • participant name
  • participant reference number or NDIS number
  • dates of support
  • support item reference number
  • support item price

Add your own name and ABN, an invoice number, the date, the hours or quantity on each line and the total — what any sole trader invoice carries. For a plan-managed participant the NDIA is specific: the invoice must include a valid ABN, unless you are exempt from quoting one — in which case you attach the ATO’s statement by a supplier form instead.

Who it goes to

  • Self-managed: to the participant, who pays you. Once they have, send them a receipt or the paid invoice so they can claim it in their portal.
  • Plan-managed: to the plan manager, who pays you and makes the claim.
  • NDIA-managed: no invoice at all. You lodge a payment request in the myplace provider portal, and you must be recorded as one of the participant’s providers first.

When an NDIS support is GST-free

Section 38-38 of the GST Act makes a support GST-free only when all four of these hold:

  1. it is supplied to an NDIS participant whose plan is in effect;
  2. it is one of the reasonable and necessary supports listed in that plan;
  3. it is supplied under a written agreement — with the participant or another person — that identifies the participant and says the supply is of supports in their plan;
  4. it is of a kind the Minister has determined in writing.

The third is the easiest to miss: no written agreement, no GST-free supply, however clearly the support sits in the plan. The fourth points to a determination that lists the kinds outright —

  • specialist disability accommodation and accommodation or tenancy assistance
  • coordinating or managing life stages, transitions and supports
  • household tasks
  • help with travel and transport arrangements, but not taxi fares
  • interpreting and translation
  • help to access and keep education and employment
  • assistive equipment for recreation
  • early intervention supports for early childhood
  • managing the funding for supports in a participant's plan

— and five more that count only if another GST-free determination, for care or health services, covers them as well: help with daily personal activities; daily living and life skills, including community participation; assistive equipment for general tasks and leisure; behavioural support and therapeutic supports; home modifications. For those, check the care or health services determination before you mark the invoice GST-free; this page has not read them and does not decide it for you.

The determination stops applying after 30 June 2027

Section 7 of the 2021 determination says it applies to supplies made on or before 30 June 2027. Unless it is extended or replaced before then, the fourth condition cannot be met for a support delivered after that date. Nothing on the Register showed a replacement when this page was checked; it is re-checked before the date arrives.

Frequently asked questions

What is a support item number?
It identifies the support in the NDIS support catalogue — the NDIA’s spreadsheet of the supports a provider can claim for, with what the NDIA considers an appropriate maximum price for each. The plan manager or the participant needs it to claim, so one goes on every line; two different supports on the same day are two lines.
Can I charge less than the NDIS price limit?
Yes. The limits are maximums, and the NDIA says in terms that you can negotiate a lower price with your participant. Put the price you actually agreed in the unit price column — the claim is for what you charged, not for the limit.
I am not registered for GST. Does that change the invoice?
Only the GST line: write that you are not registered for GST, and charge none. The four conditions for a GST-free NDIS support only matter to a provider who is registered (or required to be), because an unregistered one under the threshold is not charging GST on anything. Everything the plan manager needs to claim stays exactly the same.
How late can I send an NDIS invoice?
The NDIA takes payment requests up to two years after the support, but do not lean on that. A claim lodged more than six months after the service may be held for a review of up to 28 days, and for an NDIA-managed participant the portal request is due within 90 days of the service booking ending. Invoicing each week or fortnight keeps every claim well inside all three.
